Overview​

Pearl Hospital is seeking a motivated and experienced Community Health Programs Coordinator to lead the implementation of its community-based health initiatives. The role 

involves coordinating field activities, managing community health staff, and ensuring effective  delivery of care to clients outside the hospital setting. 

Key Responsibilities 


Plan and implement community outreach activities including community NCD and home-based care programs in collaboration with key partners 
Coordinate and supervise field teams providing health services at community and household level 
Support patient follow-up, care continuity, and reporting 
Strengthen linkages between hospital services and community health structures 


Qualifications: 


Degree or Diploma in Nursing, Community Health or related field 
Minimum 2 years’ experience in community health programming 
Strong organizational and leadership skills 
Experience managing field-based teams
