Support Planning and Implementation of community interventions to improve quality and integration of the community HIV/TB Services.
Provide coaching, Train, mentorship and technical assistance to peers who work with community, which involves Key Populations / priority populations and ensure all the KP/PP registers are up to date.
Coordinate the HIV counseling and testing, ART intiation in the community, proper follow up and accountability of testing commodities and ART drugs.
Prepare and submit monthly and quarterly work plans, reports, documentation and dissemination of community success stories to the Strategic Information team promptly.
Coordinate with peers who distribute the self-test kits and ensure they distributed in time and reports are handed over to counselors in time.
Work closely with the community teams to complete proper accountabilities of all funds released during the community activities.
Coordinate regular assessments and monitoring visits to ensure an effective referral system from community to the facility to minimize losses to linkage
Support Local radio announcements and ensure availability of IEC (information, education and communication (IEC) materials, burners for HIV free testing are in all testing points.
