  • Provide supportive supervision for optimized HIV case finding by optimizing index testing and ensuring all virally unsuppressed clients as well as newly enrolled clients are offered Index testing, have their partners tested with appropriate documentation, and reported on tools and registers. He/She to also ensure an elicitation ratio of 1:4 sexual partners.
  • Account for all the clients in the facility, and ensure effective follow-up.
  • Take an active role in case finding in the facility and community especially ensuring Linkage and Initiation on ART as well as retention of all clients.
  • Proactively tracks daily and weekly appointments as well as ensure reminders are sent to clients for drug pick-up and hospital appointment.
  • Promptly start the tracking of clients that missed appointments before they are declared Interruption-in-treatment (IIT) and document tracking outcome.
  • Engage the DEC to generate the line listing of clients IIT and ensure they are promptly tracked in the community and update the tracking outcome in appropriate tools
  • Improve client’s access to recommended HIV care and treatment services, educate and empower the clients for active involvement in their care.
  • Provide treatment education, adherence counseling, and peer support to assigned clients at health facilities and community following the adherence support schedule as well as VL optimization.
  • Oversees regular monitoring of treatment adherence, tracking defaulting/ IIT Clients and facilitate their return to care.
  • Maintains a log of all assigned clients’ contacts (descriptive addresses and phone numbers), and keeps comprehensive records of clinic appointments and interventions carried out to support them on retention.
  • Support the commencement and completion of EAC for 100% of all unsuppressed clients within 3 months
  • Assist in the monitoring of the suppression cascade across all supported sites to ensure that all unsuppressed clients eventually become suppressed.
  • Support health facility and community management of infected children and their families across supported sites.
  • Maintain confidentiality, respect privacy, and preserve the clients’ routine and independence as much as possible.
  • Ensure 100% accuracy and timeliness of documentation across all registers, LAMIS and DHIS on a daily basis.
