  • FUNCTION:
  • (1) Rendering nursing care to patient and assistance to doctors, Nurses co- workers, and managing minor cases.
  • (2) Administering prescribed medication to patients,
  • (3) Monitoring patients vital signs and ensure they are within the normal range
  • (4) Assisting in conducting minor and major theater procedures
  • (5) Monitoring and evaluating of data collected from cholera response team.
  • (6) Enrolment, Tracking, remembering patient who tested positive to HIV test for their ARV drugs pickup, tracking of lost to follow up patient and returning them to care in APIN/Faith Alive foundation.
  • (7) Working as an ART NURSE, I coordinate and oversees day to day functions of the CMT members, oversee assigned responsibilities of team members.
  • (8) Provide technical support to all team members and monitor to ensure members able to carry out designed function, oversee the development of team work.
  • (9) Identify capacity gaps and recommends/provide needed trainings,
  • (10) Planning and implementation of activities on how to prevent cholera, malaria within the community.
  • (11) Ensuring the community make use of kits such as liquid disinfection, mosquito net etc.
  • (12) Health education in schools, market and abattoirs on environmental sanitation.
  • (13) Undertaking field visit and identify with polio cases and strengthen community involvement.
  • (14) Contribute and support voluntary.
  • (15) Pre- / Post-operative Care and Intervention Care, APIN/FAF:
  • I ensure Pre-Intervention preparation are carried out on time, including stock count (order needed medication), file management and patient management
  • I support the OT team in preparations for surgical intervention; this includes preparing patients for investigations and intervention screening.
  • Supervise quality wound care for all patients on admission pre and post operatively.
  • (16) Medical Accountabilities, APIN/FAF:
  • Team Data Management:
  • I coordinate the submission of narrative for MR report on the last day of month for my team.
  • I ensure incentive assessment of incentives payment reflects the activity in the CMT, and submit early for approval.
  • I plan monthly pre-appointment list for my team to track, using the EMR.
  • I filter using the EMR data base to generate monthly and quarterly eligible patient who are due for viral load test for my viral load champion to track.
  • I ensure patients admission, follow up visits, procedures, investigations, discharge and mortality are captured in the MTF form.
  • (17)Reporting, APIN/FAF:
  • Report to APIN on monthly bases, all relevant information linked to CMT activities.
  • Set objectives for the CMT team on how to meet monthly and quarterly target and carry out monthly /end of the financial year evaluation.
  • (18) COVID-19 VACCINATOR AND COORDINTOR
  • I liaise with the local government PHC in other to make vaccine available at the facility vaccination unit
  • I ensure the recorder and the validator do their work appropriately
  • I send daily report to local government (JOS NORTH LGA), and to the NPHCDAN data base by upload vaccinated information,weekly and monthly report typed on excel format is sent to our sponsor (APIN) covid-19 vaccine focal person Through WHATSAPP group chat
  • I also liaise with the facility Team leaders, as well as the doctors, nurses, pharmacist and other health worker in the facility to advocate by encouraging patients who came to access care to get vaccinated.
